Before any new exercise session/program certain forms are required.

If embarking on Personal Training, the PAR-Q+ is for everyone, it's a short, simple form taking just a few minutes.
​
Go to the FORMS page, containing a fillable form PAR Q+ online which you can submit it, or save and bring in person or send in via email/text photo.
The Par Med X (if necessary) is required for medical purposes.
I have paper copies I can give you or you can download it on the FORMS page and print it yourself.

A Lifestyle Questionnare is found on the FORMS page or I will give you a paper copy at your assessment. It is to be completed in regards to daily activities, and any barriers you may have or be experiencing in attaining your personal goals !

Other assessments for Personal Training:
Initial heart rate monitoring with respect to cardio conditioning goals.
A posture and range of motion, if necessary, will be done after the consultation.

Any posture, range of motion, pain (and the like) issues that are OUT of my scope will be referred to an appropriate physiotherapist, kinesiologist or other licensed professional if you wish or require advisement.

Nutrition:
My PT Certification includes nutrition in the scope of the CFG; the Canada Food Guide.
Nutrition advisement will only be given within this scope. I can provide the latest nutrition information as per the CFG, coach with meal planning strategies, and teach you the breakdown of the food groups and nutrients.

I do not subscribe to the diet culture. I promote a healthy balance
80/20. Intuitive eating patterns along with mindful but not stress full
selections of your choice of fuel.
​
Any nutrition advice beyond my scope shall be instead referred (if you wish) to a local Registered Dietician or other relevant specialist.
